What is Laparoscopy?
Laparoscopy is a method by which surgeons look inside the abdomen using a telescope. Thus without opening the abdomen with a big incision the surgeon is able to diagnose many a problems and do surgery inside hte abdomen with hand held instruments.
It started with tubeligation surgery in which by making a very small incision near the umbilicus the laparoscope is introduced inside the abdomen and fallopian tubes are ligated. With gradual research and development it has become possible to do many types of surgeries, e.g. Laparoscopic Cholecystectomy (Lap Choly), Lap Appendicectomy, Lap assisted Vaginal Hysterectomy, Tubeligation, Ovariotomy, Adhesionolysis, Hernia repair etc.
What is the best method for Cholycystectomy?
Worldwide experience shows that Lap Choly is the best, though in some cases it may be wise to do open conventional surgery.
Is it possible to take out whole sac of gall bladder with the stones?
Yes, if the stones are smaller in size. When the gall stones are larger in size the stones are crushed or broken inside and removed piecemeal without damaging the gall bladde wall. Finally the gall bladder is also removed in toto.
What happens when Gall Bladder is very big?
In such cases the Gall Bladder is evacuated using a aspiration needle. Once the size is reduced the Gall bladder is removed.
What are the advantages of Lap Choly?
The incisions are small.
Resulting in less pain.
Short hospital stay.
Means early return to home and less expensive.
The surgeon is also able to see the whole abdominal cavity and other silent problems can be detected and corrected if need be.
What are the disadvantages of Lap Choly?
It requires specialised skill in Lap Surgery so al surgeons cannot do it. Further it requires costly equipments which are not usuallu available at general nursing homes and Hospitals. Further the Anaesthesiologist’s role is very important. He/she must be conversant with the technique and skilled in relaxant anaesthesia.
Sometimes due to certain problems, like extensive adhesions, mass, CBD stones, bleeding etc the surgeon may have to revert back to open surgery for the benefit of the patient. These are rare but may be needed in your case.
